Health Needs Assessment

Every three years, Union Hospital Inc. (Union Hospital Terre Haute and Union Hospital Clinton) conducts a Health Needs Assessment of the community - in partnership with key community leaders and organizations. In conducting a community health needs assessment (CHNA), each tax-exempt hospital facility must:

  • Define the community it serves;
  • Assess the health needs of that community;
  • Solicit and take into account input from persons who represent the broad interests of that community, including those with special knowledge of or expertise in public health;
  • Document the CHNA in a written report that is adopted for the hospital facility by an authorized body of the facility; and,
  • Make the CHNA report widely available to the public

 

An implementation framework, or strategy, is then developed to focus community outreach in the ways most likely to improve community health during the next 3 years. The 2019-2021 framework was approved by the Union Health and Union Hospital Inc. Boards of Directors in November of 2021.
