Our Diabetes Clinic in Terre Haute is committed to providing patients with the support they need to manage diabetes. A strong partnership between primary care providers and our team will result in the best outcomes for patients.

In order to best support the broad spectrum of diabetes patients we care for, we have a consult-to-treat model.  Upon referral to the Union Health Diabetes Clinic, patients will receive a thorough evaluation, care plan, and education. The results of that consultation will be shared with the patient's primary care provider.

We believe this process best supports our patients by creating continuity of care,  maintaining clear lines of communication with referring physicians and their role as the primary coordinator of care for their patients.

Union Hospital Diabetes Education Center and Outpatient Nutrition
Union Hospital Diabetes Education Center and Outpatient Nutrition (DEC) provides information, skills training and support to assist individuals in the management of diabetes and other nutrition concerns. Along with your physician, the staff at the DEC will help you create a diabetes management plan or other meal plan that works for you. Staff at the DEC includes Diabetes Nurse Educators and Registered Dietitians. Union Hospital Diabetes Education Center is recognized by the Diabetes Education Accreditation Program through the American Association of Diabetes Educators as meeting the National Standards for Diabetes Self-Management Education. Many of our staff are Certified Diabetes Educators.
