The Patient Navigator helps cancer patients and their families find and make use of various resources, treatment services, and support services that they need to receive the best cancer treatment possible. Services provided by patient navigators vary by the needs of the patient, but often include:
- facilitating communication among patients, family members & health care providers
- coordinating care among providers
- finding financial support and assisting with paperwork
- scheduling transportation and child care
- facilitating follow-up appointments
- ensuring the availability of medical records at appointments
The Patient Navigator helps remove any roadblocks to treatment so that patients are able to keep their appointments, follow their treatment regimen, and receive the support services they need. The goal of the Patient Navigator program is to make sure that the comprehensive and varied cancer care services offered through Hux Cancer Center and the American Cancer Society are accessible by all patients -- especially those who may not have the advantage of a strong family or community support system.
