- Checklist for mandatory activities
- Obtain Indiana Pharmacist licensure by August 15th
- Failure to obtain licensure by this date may result in a 30-day grace period.
- Failure to obtain licensure by September 15th will result in termination of the residency agreement.
- Obtain BLS, ACLS, and PALS certification
- Complete of a minimum of three (3) patient case presentations
- Complete of a minimum of one (1) medication use evaluation (MUE)
- Complete of a minimum of one (1) drug monograph
- Complete a minimum of two (2) journal club presentations
- Complete two (2) pharmacy newsletters
- Complete project plans for MUE and residency project
- Present residency project at the Union Hospital Continuing Medical Education (CME) Grand Rounds
- Present residency project at the Great Lakes Residency Conference
- Complete residency project manuscript
- Obtain teaching certificate from the Indiana Pharmacy Teaching Certificate (IPTeC) Program
- Complete Resident Portfolio (stored on pharmacy shared G drive)
- 90% required objectives evaluated and scored as “Achieved”
Please note that the above are minimum requirements for successful completion of the program. Residents will be given ample opportunity to meet the above required activities.
